Job Description The Crox Group is seeking a local contract nurse RN Care Manager for a local contract nursing job in Eureka, California. Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Duration: 12 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Job Description Summary: Provide comprehensive case management services and coordinate patient care plans across the continuum, ensure safe and efficient transitions, and collaborate with interdisciplinary teams to optimize outcomes for a diverse acute-care population. Responsibilities: - Conduct assessments to identify patient needs, barriers, and resources - Develop and implement individualized discharge and care transition plans - Collaborate with physicians, nurses, social workers, and community providers - Monitor patient progress, adjust plans as needed, and document in Epic - Facilitate referrals to home health, durable medical equipment, and outpatient services - Educate patients and families on disease management, community resources, and follow-up care - Work every other weekend and commit to at least three of the facility’s observed holidays Qualifications Education: - Graduate of an accredited Bachelor of Science in Nursing (BSN) program Experience: - Minimum 1 year of acute-care Registered Nurse experience - Prior case management or utilization review experience preferred License: - Active California RN license in hand at submission (no temporary or pending) - Basic Life Support (BLS) certification Skills: - Proficient in Epic EMR documentation and care-coordination tools - Strong assessment, clinical judgment, and discharge-planning abilities - Excellent communication and interdisciplinary collaboration skills - Knowledge of community resources and insurance/utilization review processes - Ability to manage multiple complex cases and meet regulatory requirements About The Crox Group A firm commitment towards bringing an impactful change in business domains enables us to explore and innovate continuously.
