provided to qualified applicants Schedule: Full-time; every other
weekend required; caseload up to 25 patients; no call required

Minimum 2 years of acute care hospital case management experience

POLST, and protective services processes Proficiency with Epic
charting system required Skills in discharge planning, DRG coding,
appeals and denials, prior authorizations, and care coordination
Understanding of CMS guidelines, HIPAA, and Joint Commission
standards Minimum of 2 supervisory references covering 1 year of
