Registered Nurse (RN) | Case Manager Location: Eureka, CA Agency: Lancesoft Inc Pay: $2,800 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position The RN Case Manager coordinates comprehensive patient care to ensure quality outcomes and efficient care transitions across the continuum of care. This role focuses on assessment, discharge planning, utilization review, and patient advocacy. The RN collaborates with physicians, social workers, insurers, and community agencies to optimize patient safety and satisfaction.
