Type: Contract Shift Type: Day shifts, 8-hour shifts (exact
start/end times vary by unit), typically Monday-Friday with Every
Other Weekend (EOW) coverage, First-to-Float (floats to all units
excluding surgery) Hours per Week: 40 Candidate Pay Rate: $82 per

Responsibilities Coordinate patient care across teams to ensure
safe, timely transitions Perform case management and utilization
review to support appropriate care delivery Manage a patient
caseload typically of 15 to 25 patients per day, adjusting as
staffing requires Document patient interactions and care plans
accurately using Cerner Serve as the first float to other units
when needed, providing adaptable nursing support Collaborate with
multidisciplinary teams including physicians, social work and
ancillary services Provide clear communication with patients and
families about care coordination and discharge plans Participate in
required orientation and ongoing competency updates Qualifications
Active Registered Nurse license in California BLS certification
required Minimum of 2 years of clinical nursing experience Recent
experience as a Care Coordinator at a California Dignity Health
hospital or CommonSpirit facility is required for consideration Two
supervisory references required at submittal (one within 12 months
and one within 3 years); charge accepted Candidates not employed by
a CommonSpirit-associated facility within the past year are
preferred; PRN employment through outside firms may be considered
Comfort floating to multiple acute care units and managing a
diverse patient population including cardiac, surgical, oncology
and critical care Education Associate Degree in Nursing (ADN) or
Bachelor of Science in Nursing (BSN) acceptable; BSN not required
Active California RN license Knowledge & Skills Strong knowledge of
acute care nursing principles and care coordination Experience with
case management and utilization review processes Proficiency in
Cerner charting and electronic documentation Excellent clinical
judgment, prioritization and time management skills Effective
verbal and written communication with interdisciplinary teams
Ability to handle a moderate to high patient volume while
maintaining quality care Familiarity with discharge planning,
