Registered Nurse (RN) | Case Manager Location: Sacramento, CA Agency: Thrive Staffing Pay: $2,838 to $3,032 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/3/2026 About the Position Job Title RN (Registered Nurse), Case Manager City/State: Sacramento, California Shift: 8 Hour Days, 08:00:00-16:00:00, EOW, First to Float Contract Length: 13 weeks Start Date: 03/03/2026 Job Description: The RN Case Manager will oversee a caseload of 15-25 patients per day, providing specialized care coordination across all units except surgery within an acute care hospital setting. Candidates must have recent experience as a Care Coordinator in a California Dignity Health Hospital. Responsibilities: Manage and coordinate patient care for a diverse population, including cardiac, surgical, oncology, and critical care units. Adapt to various departmental needs, except surgical departments, as the first to float. Requirements: BLS Certification Must have recent California Dignity Health Hospital experience as a Care Coordinator.
