Job Description:

New locum open for IM/FM in Eureka, CA- 30 day credentialing-$165 Hr-June Start-CA license only to apply $165 hr Monday-Friday 8-5 Mix of peds/adults A decision is typically made within 7-14 days We will also accept DO BE will be considered if truly BE in FM, IM, or Peds (5 years or less since residency) Active DEA needed before start Credentialing takes 30-90 days Candidate CVs need to have month & date on their work experience w/all gaps in employment accounted for. Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD The following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30 day notice for ending all assignments.
