Wage for 20 hours/week is up to ~ $80,000 Up to $50,000 available
to individuals outside of Yolo county, Solano County, and PHPs

you were a Family Nurse Practitioner or Physician Assistant with
Winters Healthcare, you would be one of our five care team
providers, operating independently amongst a primary care team at
our facility in Esparto or Winters. Team [ insert your name here ],
includes a compassionate Medical Assistant, a motivating Population
Health Coach (PHC), a designated Patient Service Coordinator (PSC)
working side-by-side for your panel of patients, as well as
generative AI technology for supporting seamless documentation of
encounters, easing the administrative workload. Additionally, you
will work alongside our Clinical Psychologists for warm hand-offs
and other behavioral health consultation and support. To enhance
our integrated care teams, and resources for patients, we also have
our dental department right across the lobby, as well as an on-site
pharmacist and pharmacy. Our services include full-scope family
practice: adult and pediatric primary care, chronic disease
management, prenatal care, and minor procedures. Your day will vary
from visit to visit: an 18-month-old well-child visit, to a
63-year-old uninsured individual with a cough, to a diabetic
migrant-seasonal farm worker, or a 25-year-old coming in with their
first pregnancy. Each care team typically sees around 17 20
patients in an 8-hour shift, and have patient panels that average
around 700. We place emphasis on establishing trusting
relationships while being devoted to quality health outcomes and we
have the data and measures results to prove it. The ideal candidate
is a bilingual (English/Spanish) Family Practice Provider (MD, FNP,
or PA) who provides prenatal care up to 36 weeks. Additionally, our

Communicate, collaborate, and work in a team-oriented environment
supporting our model of care Provide full-scope family medicine,
chronic disease management, minor surgeries/procedures, prenatal
care (low risk OB), family planning, and reproductive health
Provide direction and supervision of clinical services; assure
quality, continuity, and appropriateness of services provided.
Consult with care teams as needed, and act as supervising
physician, providing Medical Doctor oversight for mid-level
providers Qualifications: Current FNP or PA license to practice in
the state of California, as well as valid DEA, and CPR/ACLS/BLS;
Minimum of 5 years clinical experience, with comfort to be the only
medical provider on site at varying times throughout the week.
Bilingual fluency in English/Spanish is preferred (ability to read,
write, and speak), Board eligible or board certified (within 6
months of hire), Strong leadership skills and dedication to serving
community health care are essential. Compensation and Benefits:
Annual equivalent salary ranges are dependent on experience:
$137,800 - $160,960 for 32 hours/week (our full-time) & based on
qualifications and experience Individuals working less than 24
hours per week will be paid on an hourly basis, with a range of $66
- $77 DOE Continuing Medical Education reimbursement and time off

We are a loan repayment eligible site with NHSC, OSHPD/HCAI, and
CalHealth Cares! NP and PA candidates who are outside of the
Partnership Health Plan 14 counties are eligible for a $50,000
bonus for a full-time commitment ($25,000 for half-time) We promote
a work-life balance no scheduled shifts over 8 hours and we are
closed on the weekends. For individuals working 24-31 hours per

Practitioner or Physician Assistant Reports to: Director of
Clinical Services/ Medical Director Summary of Duties The Family
Nurse Practitioner (FNP) or Physician Assistant (PA) works within a
model of team-based care with the care team and is responsible for
full-scope family practice services including prenatal, and family
planning services. Under the supervision of the Medical Director,
the FNP will perform complete examinations of patients and screen
all patients for health maintenance needs, including immunizations
on an integrated healthcare team providing relationship-based model
of care. The FNP will document clinical observations, adhere to
industry standards and conduct chart reviews to ensure compliance.
Essential Functions: Responsible for providing primary healthcare
services to patients and families focusing on healthcare
maintenance, disease prevention, patient education and counseling
Performs health assessments, CHDP physicals, gynecological
procedures, orthopedic procedures, provides urgent care and
performs emergency procedures Performs basic lab testing, including
interpretation of results. Orders appropriate pharmacological
interventions Actively participates in quality assurance and
quality improvement efforts and process improvement programs
Participates in meetings and completes administrative tasks as
assigned Assists in the recording of no-show visits, lab results,
and prescription refill requests Support quality improvement and
performance improvement efforts, and participate in related
activities as required. Performs other duties as assigned Minimum
Requirements: Education and Work Experience Current license to
practice as an FNP or PA in the State of California. Minimum two
years experience in a public health arena, preferably in a
community clinic setting BCLS, ACLS, or CPR Certification (or
