standard for Locums who submit app./documents timely)The following
immunizations are required for credentialing (before start date):
2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be
within the 90 days before start date), Positive MMR Titer or
Complete Immunization Series, Positive Hep B Titer or Complete
Immunization Series, Positive Varicella Zoster Titer or Complete
Immunization Series, Tdap Immunization (must be within the last 10
years).HCO & Vendor agree to a 30-day notice for ending all
